By booking any appointment with CiviCore Collection & Screening Services (“CiviCore”), you acknowledge, understand, and agree to the following Terms & Conditions. These terms govern all services provided, including drug testing, alcohol testing, DNA specimen collection, fingerprinting, and all related mobile and in-office services.

1. Scope of Services
CiviCore provides specimen collection, testing facilitation, notary-related services, and other administrative services.
CiviCore does not interpret test results, diagnose medical conditions, or provide legal or medical advice.

2. Booking Requirements
Clients must book appointments at least 4 hours before the scheduled start time.
Clients may book appointments up to 14 days in advance, unless otherwise noted for special services.
Appointments are subject to availability and confirmation.

3. Client Responsibilities
By booking, you agree to:

Provide accurate personal information and identification

Arrive on time with required documentation

Follow all testing procedures and instructions
-
Disclose any relevant information needed to complete services
-
Treat staff with respect and professionalism
Failure to comply may result in refusal of service or termination of the appointment.

4. Payments & Deposits

Some services require a non-refundable deposit at booking
-
Deposits secure your appointment and are applied to the final balance
-
Deposits are non-refundable under all circumstances
-
Full payment for services is due at the time of service unless otherwise arranged

Service fees, mobile fees, or after-hours fees may apply when applicable

5. Rescheduling Policy

You may reschedule appointments with at least 24 hours’ notice
-
Rescheduling requests made less than 24 hours before the appointment may require a new deposit
-
All rescheduling is subject to availability

6. Cancellation & No-Show Policy
To ensure fairness to all clients:

Deposits are non-refundable regardless of cancellation time
-
Cancellations made less than 24 hours before the appointment will incur a $50 cancellation fee
-
“No-show” or “no-call/no-show” appointments will incur a $50 fee
-
No-call/no-show clients must pay for a new test in full before being allowed to rebook
-
Fees must be paid before a new appointment will be confirmed

7. Late Arrivals

A 15-minute grace period is allowed
-
Clients arriving more than 15 minutes late may be marked as a no-show
-
Service may need to be rescheduled depending on availability
-
Late arrivals may still be charged applicable fees

8. Identification Requirements
Clients may be required to present one or more of the following:

Government-issued photo ID

Employer-issued documents
-
Authorization forms
-
Court, employer, or agency paperwork
Failure to provide required documentation may result in appointment cancellation without refund.

9. Privacy, Consent & Confidentiality

CiviCore complies with all industry privacy standards

Your personal information will only be used to complete services
-
Information is not shared with third parties except when required by law or the employer/agency requesting the test
-
You consent to the collection and transmission of specimens or information as required by the relevant testing agency or laboratory
SMS & Email Communications
By providing your mobile phone number or email address and booking services with CiviCore, you consent to receive appointment confirmations, reminders, service notifications, and customer support messages via SMS text message and email.

Messages may include:

Requests for additional information

Follow-up communications
Message Frequency
Message frequency varies depending on your activity, but clients may receive up to 4 messages per appointment or up to 4 messages per month.
Message and Data Rates
Message and data rates may apply depending on your carrier.
Opt-Out Instructions
You may opt out of SMS communications at any time by replying:
STOP
You may opt out of email communications by replying UNSUBSCRIBE or by contacting us directly.
(Essential communications such as appointment confirmations cannot be opted out of.)

Data Sharing Restrictions
CiviCore does not share, sell, rent, or transfer your personal information or mobile phone number to third parties for marketing or promotional purposes under any circumstances.
We only share the minimum necessary information with trusted service providers (e.g., laboratories, scheduling tools, payment processors) to complete your service.

10. Accuracy of Information
You agree that:

All information you provide is true and correct

CiviCore is not responsible for delays or errors resulting from incomplete, inaccurate, or fraudulent information

11. Test Results
CiviCore:

Does not interpret or explain test results

Is not responsible for laboratory delays or errors

Is not liable for decisions made by employers, agencies, or third parties based on test results
All results are issued directly by the authorized laboratory or agency according to their guidelines.

12. Right to Refuse Service
CiviCore reserves the right to refuse service to any client for:

Aggressive or inappropriate behavior

Fraudulent or suspicious activity

Safety concerns
-
Non-payment or unpaid past fees
-
Failure to follow instructions or policies

13. Liability Limitations
CiviCore is not liable for:

Lost wages or employment decisions

Court or agency outcomes
-
Delays caused by laboratories or external entities
-
Client failure to follow instructions
-
Damages arising from the use or interpretation of test results

14. Changes to Terms
CiviCore may update or modify these Terms & Conditions at any time.
The most current version will always be available on our website and/or booking system.

15. Governing Law
These Terms & Conditions are governed by the laws of the State of Georgia.
By booking, you agree that any disputes shall be resolved under Georgia state law.
